– The capitalized first letter of your first name
- The first letter of your Middle name
-The first letter of your last name and
- Your six-digit birth date

For example, if your name is
 John Marco Smith and you were born on January 1ST, 1981,
your login would be Jms010181



**If you don’t have a middle name use the second letter of your first name

For example, if your name is
John Smith and you were born on January 1ST, 1981,
your login would be Jos010181

Your temporary PASSWORD is: Password1

You must change your password the first time you login. Your new password minimum requirements are at least 7 characters that includes1 capital letter and 1 number.

This portal can be used to: 
Review and verify your contact information, medical history, medications and allergies, communicate with your provider and/or their staff and request prescription refills.
